Replacement Code digits?

    I have a digit list on my computer. However, there's a video at YouTube where AntiWeapon fights against 6 Terras, and there's the codes in the description. They were like this:
    Save point mod for the hall of the cornerstone:
    11C533E8 0000????

    Mod Chip:
    11C53894 0000????

    Mod Dale:
    11C53AD4 0000????

    Skateboard mod:
    11C535C4 0000????

    However, when I look up to the digit list on my computer, Dale isn't AD4, it's 492.
    What's the meaning of that? I just want to replace the portal to Terra fight with Terra, and the door to the Timeless River with Terra...
    If anyone knows where to find the appropriate digit list, please post here or PM me.

    I think a Coder is needed for this, because these codes are wrong.

    I have that list already, but for example, what is the code if I want to replace all Shadows with Terras or Sephiroths? And I think the list is for only output, not input. You can find Terra there, but if you want to replace Terra, the first 4 digits wouldn't be 096F. The code would be
    Terra replaced by
    11C556E0 0000????

    I don't know how to really explain it.
